Once you’ve completed the form, please save it and return by email. The form must not be signed and faxed, nor must it be converted into any other format. 

At the time of acceptance of the quotation, you’ll need to sign a declaration confirming that the information on the proposal form is correct.

If there’s no specific form for your profession, please complete the Errors and Omissions proposal form.
